Abstract

PURPOSE:

This study investigated whether pedicle screws medially misplaced into the spinal canal without neurological complications should be removed or not.

METHODS:

A total of 86 patients with scoliosis that underwent spinal fusion using 988 pedicle screws were retrospectively reviewed after a minimum follow-up of 2 years. The inclusion criteria were: (1) patients without outstanding problems during the insertion of pedicle screws, (2) patients without neurological deficits either intraoperatively or postoperatively, and (3) patients that had all implants removed after bone union upon the request of the patient. Medial perforations were evaluated using immediate postoperative helical CT images and classified into three grades: grade 1 (0-2 mm), grade 2 (2-4 mm), and grade 3 (over 4 mm). All unexpected events were recorded at the time of removal.

RESULTS:

CT images obtained 2 years postoperatively exhibited neither loosening of screws nor pseudoarthrosis in all patients. CSF leakage from screw holes were recognized in 3 of 87 medially misplaced screws (3.4 %). There was no CSF leakage in grade 1 (35 screws), one CSF leakage (2.5 %) in grade 2 (40 screws), and two (16.7 %) in grade 3 (12 screws). No neurological abnormalities occurred either intraoperatively or postoperatively.

CONCLUSION:

This study indicated that screws medially misplaced at a distance greater than 2 mm, especially 4 mm, may be a cause of negative effects on the neural structure and should be removed during the early phase of the postoperative period, even among patients without postoperative neurological abnormalities.

ResumenPROPÓSITO:
Este estudio investigó si los tornillos de pedículo medial fuera de lugar en el canal espinal sin complicaciones neurológicas deben retirarse o no. 
MÉTODOS:Un total de 86 pacientes con escoliosis que se sometieron a la fusión espinal utilizando 988 tornillos de pedículo se revisaron retrospectivamente después de un seguimiento mínimo de 2 años. Los criterios de inclusión fueron: (1) pacientes sin problemas de circulación durante la inserción de tornillos de pedículo, (2) los pacientes sin déficit neurológico, ya sea intraoperatorio o postoperatorio, y (3) los pacientes que tenían todos los implantes retirados después de la unión ósea, a petición de la paciente . Perforaciones mediales fueron evaluados utilizando imágenes de la TC helicoidal postoperatorio inmediato y clasifican en tres grados: grado 1 (0-2 mm), grado 2 (2-4 mm), y grado 3 (más de 4 mm). Todos los eventos inesperados se registraron en el momento de la extracción. 
RESULTADOS:Las imágenes de TC obtenidas 2 años después de la operación mostraron ni aflojamiento de tornillos ni pseudoartrosis en todos los pacientes. CSF fugas de agujeros de los tornillos se reconoce en 3 de 87 tornillos medialmente fuera de lugar (3,4%). No hubo pérdida de LCR en el grado 1 (35 tornillos), la fuga de un CSF (2,5%) en el grado 2 (40 tornillos), y dos (16,7%) en el grado 3 (12 tornillos). No hay alteraciones neurológicas ocurrieron sea intraoperatorio o postoperatorio. 
CONCLUSIÓN:Este estudio indicó que los tornillos medialmente fuera de lugar a una distancia mayor que 2 mm, especialmente 4 mm, puede ser una causa de efectos negativos sobre la estructura neural y deben eliminarse durante la fase temprana de la postoperatorio, incluso entre pacientes sin anormalidades neurológicas postoperatorias .

Abstract

BACKGROUND AND OBJECTIVE:

Percutaneous vertebroplasty (PVP) has been successfully used in the treatment of pain related to osteoporotic vertebral fractures refractory to medical therapy, especially in the treatment of acute factures. However, the effectiveness of this therapeutic approach in the treatment of painful chronic vertebral fractures is less clear.

PATIENTS AND METHODS:

In this report we evaluate the short and long-term effectiveness in pain relief of PVP in a group of 5 patients with pain related to chronic osteoporotic vertebral fractures without bone marrow edema (BME) on magnetic resonance imaging (MRI). All patients were followed during one year, assessing analgesic use, pain evolution (on a 10-point visual analog scale [VAS]), new vertebral fractures and other clinical complications. Seven procedures were performed in the 5 patients.

RESULTS:

All patients reported substantial improvement in back pain 2 weeks after the procedure, with a mean decrease of 53% in the VAS. However, one year after PVP most patients (4 out 5) worsened, achieving similar VAS scores to those obtained at baseline. No additional vertebralfractures or other clinical complications were observed.

CONCLUSION:

The present cases suggest that the long-term effectiveness of PVP in the treatment of painful chronic vertebral fractures without BME on MRI is scarce.


Resumen

Fundamento y objetivoLa vertebroplastia percutánea (VPP) ha sido utilizada con éxito en el tratamiento del dolor secundario a fracturas vertebrales osteoporóticas refractario a terapia conservadora, especialmente en el tratamiento de fracturas agudas. Sin embargo, la eficacia de este procedimiento en el tratamiento de fracturas vertebrales crónicas no está clara.
Pacientes y métodoEn esta serie de casos clínicos evaluamos la eficacia a corto y largo plazo de la VPP en el alivio del dolor asociado a fractura vertebral crónica osteoporótica sin edema en la resonancia magnética (RM), en un grupo de 5 pacientes. Se realizó un seguimiento de un año a todos los pacientes, se evaluó el consumo de analgésicos, la evolución del dolor (en una escala visual analógica [EVA] de 0 a 10 puntos), nuevas fracturas vertebrales y otras complicaciones clínicas. Se realizaron 7 procedimientos en 5 pacientes.
ResultadosTodos los pacientes refirieron una mejoría sustancial del dolor a las 2 semanas del procedimiento, con una media de disminución de la EVA del 53%. Sin embargo, un año después de la VPP la mayoría de los pacientes (4 de 5) empeoró, alcanzando niveles de EVA similares a los basales. No se observaron nuevas fracturas vertebrales ni otras complicaciones clínicas.
ConclusionesEstos casos sugieren que la eficacia a largo plazo de la VPP en el tratamiento de las fracturas crónicas sintomáticas sin edema en la RM es escasa.
