Abstract
The purpose of this clinical practice guideline is to help improve treatment and management of hip fractures in the elderly based on current best evidence. The guideline contains twenty-five recommendations, including both diagnosis and treatment. Of those recommendations, strong evidence supports regional analgesia to improve preoperative pain control, similar outcomes for general or spinal anesthesia, arthroplasty for patients with unstable (displaced) femoral neck fractures, the use of a cephalomedullary device for the treatment of patients with subtrochanteric or reverse obliquity fractures, a blood transfusion threshold of no higher than 8 g/dL in asymptomatic postoperative patients, intensive physical therapy postdischarge, use of an interdisciplinary care program in patients with mild to moderate dementia, and multimodal pain management after hip fracture surgery. In addition to the recommendations, the work group highlighted the need for better research in the treatment of hip fractures.

El propósito de esta guía de práctica clínica es ayudar a mejorar el tratamiento y manejo de las fracturas de cadera en los ancianos con base en la mejor evidencia disponible. La guía contiene veinticinco recomendaciones, incluyendo el diagnóstico y el tratamiento. De esas recomendaciones, una fuerte evidencia apoya la analgesia regional para mejorar el control preoperatorio dolor, resultados similares para la anestesia general o raquídea, artroplastia para los pacientes con inestables (desplazadas) fracturas del cuello femoral, el uso de un dispositivo cefalomedular para el tratamiento de pacientes con subtrocantérea o inversa fracturas oblicuidad, un umbral de transfusión de sangre no superior a 8 g / dl en los pacientes postoperatorios asintomáticos, intensiva después del alta de terapia física, el uso de un programa de atención interdisciplinar en pacientes con demencia leve a moderada, y el manejo del dolor multimodal después de la cirugía por fractura de cadera. Además de las recomendaciones, el grupo de trabajo puso de relieve la necesidad de mejorar la investigación en el tratamiento de las fracturas de cadera.

Abstract
OBJECTIVES:
To evaluate the radiographic and clinical outcomes of a systematic approach to exchange nailing for the treatment of aseptic femoral nonunions previously treated with an intramedullary nail.
DESIGN:
Retrospective cohort.
SETTING:
Tertiary referral center.
PATIENTS:
Fifty aseptic femoral nonunions in 49 patients who presented with an intramedullary nail in situ an average of 25 months after the initial fracture nailing were evaluated.
INTERVENTION:
Our systematic approach includes inserting an exchange nail at least 2 mm larger in diameter than the in situ nail, using a different manufacturer's nail, static interlocking, correction of any metabolic and endocrine abnormalities, and secondary nail dynamization in cases showing slow progression toward healing.
MAIN OUTCOMES MEASUREMENTS:
The outcome measures were radiographic and clinical evidence of nonunion healing and time to union.
RESULTS:
All 50 femoral nonunions (100%) healed after this systematic approach to exchange nailing. The average time to achieve union was 7 months (range, 3-26 months).
CONCLUSIONS:
Utilization of this systematic approach of exchange nailing for the treatment of aseptic femoral nonunions resulted in a 100% healing rate.

DISEÑO:
Cohorte retrospectiva.
CONFIGURACIÓN:
Centro de remisión terciario.
PACIENTES:
Cincuenta nonunions femorales asépticos en 49 pacientes de quienes presentó con un clavo intramedular in situ un promedio 25 meses después de que la enclavación inicial de la fractura fue evaluada.
INTERVENCIÓN:
Nuestro abordaje sistemático incluye la inserción de un clavo del intercambio por lo menos 2 milímetros de más grande en diámetro que el clavo in situ, usar el clavo de un diverso fabricante, enclavijarse estático, la corrección de cualquier anormalidad metabólica y endocrina, y la dinamización secundaria del clavo en los casos que muestran la progresión lenta hacia la cura.
MEDIDAS PRINCIPALES DE LOS RESULTADOS:
Las medidas de resultado fueron la evidencia radiográfica y clínica de la cicatrización de la seudoartrosis y tiempo de consolidación
RESULTADOS:
Las 50 pseudoartrosis femorales (100%) curaron después de este abordaje sistemático para intercambiar la enclavación. El tiempo promedio de alcanzar la consolidación fue de 7 meses (rango, 3-26 meses).
